- Current Medication List
- Current Lab Reports (Hematology, Chemistry, & other Lab Data)
- Current HbsAg /Hepatitis Profile
- Copies of Insurance cards (front & back) (Primary & Secondary)
- Local Address and Phone Number
- Complete Hemodialysis Admission form (Enclosed)
- Chest X-Ray
- EKG copy
